dever&atilde;o novamente preencher formul&aacute;rio de triagem.\r\nPara cancelar sua consulta e agendar nova data, &eacute; necess&aacute;rio avisar com anteced&ecirc;ncia m&iacute;nima de 6 (seis) horas.\r\nToler&acirc;ncia m&aacute;xima de 10 (dez) minutos de atraso para consulta agendada, ap&oacute;s esse per&igrave;odo, ser&aacute; considerado falta.<br>\r\n2 (duas) faltas no mesmo
